CCN Digidagboek

U heeft een holter meegekregen om uw hartritme te registreren. Daarnaast is u gevraagd om uw activiteiten en klachten bij te houden in een dagboekje. Deze CCN app ondersteunt u hierbij. U selecteert een activiteit en bijbehorende klacht; de CCN digidagboek app vult zelf de datum en tijd in.

Zo werkt het:

STARTEN

- Download en installeer het ‘CCN Digidagboek’ via de app-store op uw smartphone.

- Scan de QR-code die u van uw verpleegkundige hebt ontvangen

- U kunt het CCN Digidagboek meteen gebruiken

INVOEREN

- Open de app door het icoontje aan te tikken

- Tik ‘nieuwe invoer’ aan.

- U ziet 4 buttons: datum, tijdstip, activiteit en klacht.

- Als u de klacht meteen invult op het moment dat u het ervaart, hoeft u datum en tijd niet in te vullen. De datum en tijd staan automatisch op het moment dat u een nieuwe activiteit en klacht opent. Als u deze niet meteen op het moment van de klacht invult, kunt u deze aanpassen.

- Tik op de button activiteit: tik de activiteit aan die u aan het doen was

- Tik op de button klachten: tik de klacht aan waar u tijdens deze activiteit last van had.

- Indien u de activiteit en klacht niet meteen heeft ingevuld kunt u datum en tijd aanpassen door datum aan te tikken en de gewenste datum te selecteren. Vervolgens selecteert u de tijd waarop u de klacht had.

OPSLAAN

- Nadat u de activiteit(en), klacht(en) heeft ingevuld, en eventueel datum en tijd aangepast, tikt u op opslaan.

- U krijgt nu een overzicht van uw persoonlijk dagboekje te zien. Deze kunt u ook inzien door op ‘overzicht’ te drukken

- Tik eventueel op ‘nieuwe invoer’ om meer klachten en activiteiten in te voeren.

You have been given a holter to register your heart rhythm. You have also been asked to keep a record of your activities and complaints in a diary. This CCN app supports you in this. You select an activity and associated complaint; the CCN digidagboek app fills in the date and time itself.

That is how it works:

START

- Download and install the "CCN Digidagboek" via the app store on your smartphone.

- Scan the QR code that you have received from your nurse

- You can use the CCN Digidagboek immediately

ENTER

- Open the app by tapping the icon

- Tap on "new entry".

- You see 4 buttons: date, time, activity and complaint.

- If you immediately fill in the complaint when you experience it, you do not have to enter the date and time. The date and time are automatically stated when you open a new activity and complaint. If you do not complete this immediately at the time of the complaint, you can adjust it.

- Tap the activity button: tap the activity that you were doing

- Tap on the complaints button: tap on the complaint that you had during this activity.

- If you have not filled in the activity and complaint immediately, you can adjust the date and time by tapping date and selecting the desired date. Then select the time you had the complaint.

SAVE

- After you have entered the activity (s), complaint (s), and possibly adjusted the date and time, tap save.

- You will now see an overview of your personal diary. You can also view this by clicking on 'overview'

- If necessary, tap on "new entry" to enter more complaints and activities.
